The epinephrine used in the Epi Pen used to treat the anaphylactic shock that is currently being talked about in the news a lot is quite an interesting drug.

It is one of the catecholamines (catechol = ring structure with two hydroxyl groups) and can be both a hormone and neurotransmitter.

It works as an α-agonist at high doses and as a β-agonist at low doses. Thus, at high doses it causes vasoconstriction, which increases the blood pressure (both systolic and diastolic). Likewise when acting as a β1 agonist, it increases the heart rate and its contractility. When it works as a β2 agonist, it relaxes vascular tissue and respiratory smooth muscle to result in brochodilation and a slight decrease in diastolic pressure.

Because of its characteristics, it’s used to treat cardiac arrest and severe hypotension as well as bronchospasm and anaphylaxis. It can also be used to treat open-angle glaucoma.

最近、ニュースでよく聞く『アナフィラキシーショック 』を治療する『エピペン』に使われる『エピネフリン』は面白い薬。


高用量ではαアゴニストとして活躍、低用量では βアゴニストになる。従って、高用量では血管収縮により、血圧(収縮期血圧 と拡張期血圧)の増加が得られる。β1 アゴニストに活躍する時、心拍数と心収縮性を増す。 β2アゴニストに活躍している時、気管支を拡張させながら拡張期血圧を少し下げる。

それらの特徴から心不全重症低血圧 の治療 に使われる。急性気管支けいれんアナフィラキシー治療にも利用されている。開放隅角緑内障治療にも使われる。
