You might have wondered why doctors always want to know which meds you’re taking before they prescribe something. Most people think that drugs act independently and will do only what they’re supposed to do. A few years ago in a pharmacology course, I learned that this was far from true. Drugs can interact in the following ways.
Types of Drug Interactions
Additive
The effect of substance A and substance B together is equal to the sum of their independent individual effects. I think of this as A+B =[A][B].
An example would be aspirin and acetaminophen.
Permissive
Substance A has to be present to get the full effect of substance B. I think of this as B dependent on A.
An example would be the catecholamine response dependent on cortisol.
Synergistic
Substance A and substance B together result in a greater effect than their separate individual effects.
An example would be Clopidogrel with aspirin.
Tachyphylaxis
Dramatic decrease in response to a drug after initial/repeated administration.
Examples would be the nitrates and niacin. Hallucinogens like LSD, mescaline and psilocybin are also examples.
All of these interactions result in different results in different directions, so doctors are right to be cautious.
薬物間相互作用
医院に行くと先生が薬を処方する前に現在の服用薬を確認する。一般の人は薬は別々で働き、単独で効くと思うが、数年前に受けた薬学のコースでこれはかなり危ない考え方に気付いた。
薬品は次のような種類の間相互作用を出せる。
相加効果
これは一番解りやすい。物質Aと物質Bを足せば、別々の効果の合計が得られる。これをA+B =[A][B]で覚えている。
例:アスピリン + アセトアミノフェン
許容作用
物質Bの完全効果を得るには物質Aが必要。つまり、BがAに頼っている。
例:カテコールアミン反応のコルチゾール頼り
相乗効果 作用
物質Aと物質Bを合わせば、別々の効果よりも増した効果が得られる。
例:クロピドグレル +アスピリン
速成耐性作用
薬物の1回/複数回の投与に対しての著しい効果低下。
例:硝酸塩、ナイアシン 、LSD
上記のような間相互作用があるので注意が必要。
