Transmission of E. coli as a disease is usually via the fecal-oral route. Food and water often provide transport for infections.

There are five types of E. coli infections that have been identified so far: enterotoxigenic (ETEC); enteropathogenic (EPEC); enterohemorrhagic (EHEC); enteroinvasive (EIEC); and enteroaggregative (EAEC).

1. Enterotoxigenic (ETEC). This is one of the causes of infamous “traveler’s diarrhea” accounting for about 75% of the cases. The route of transmission is either through contaminated food or water or by human to human contact.

In this form of the disease, the bacteria’s pili bind the bacteria to the intestinal mucosa. Endotoxins force the prolonged hypersecretion of chloride ion and water by the mucosal cells and inhibit the reabsorption of sodium. This results in the intestines becoming full of water creating a watery diarrhea that continues for several days. The heat stable ST endotoxin causes an elevation in cyclic guanosine monophosphate (cGMP) levels, while the LT heat-labile endotoxin (similar to the cholera toxin) results in increased cyclic adenosine monophosphate (cAMP) levels. The end result is an osmotic diarrhea.

今まで5種類のE. coli感染が確認されている: 腸毒性大腸菌 enterotoxigenic (ETEC);腸病原性大腸菌enteropathogenic (EPEC); 腸出血性大腸菌enterohemorrhagic (EHEC); 腸侵入性大腸菌enteroinvasive (EIEC); 及び 腸管凝集性大腸菌 enteroaggregative (EAEC)

1. 腸毒性大腸菌 enterotoxigenic (ETEC)。これは最悪の旅行者下痢症の1つの病原菌で、約75%の発生はこの大腸菌から発生する。感染経路は菌に感染された食事・水または人から人への接触。

腸毒性大腸菌のリ線毛でバクテリアが粘膜に引っ付く。内毒素により粘膜細胞が過剰に塩化物イオンと水分を分泌し、ナトリウムの再吸収を抑制。その結果は数日間継続する水分の多い下痢となる。耐熱性ST内毒素がサイクリック・グアノシン一リン酸(cGMP)を上昇させると共に易熱性LT内毒素(コレラ毒素に似ている)によって環状アデノシン一リン酸(cAMP)レベルも上昇。最終的な結果は浸透圧性下痢だ。
